Referral Process:

​

To initiate the referral process, we require the following information to ensure we can provide the best care for you or your loved one. Please schedule an appointment with your physician to obtain the necessary documents. Once updated documents are available, your physician can fax them to us at (515) 500-6071.

Required Information:

• Demographics (including Social Security Number) & Copies of Insurance Cards

• Physician Orders

• Updated Medication List

• Progress Notes

• History & Physical

• Wound Assessment

• Psychiatric Assessment

• Therapy Recommendations

• PASRR Evaluation

Upon receipt of these documents, our Clinical Team will review them to confirm our ability to provide the required care. If additional information is needed, Julie Johnson, our Admissions Coordinator, will contact you or a family member. We will discuss financial options (Medicare, Medicaid, Private Pay), address any inquiries, and guide you through the admission process.

Admission Process:

Upon acceptance to our nursing community, we will schedule a date and time for you to furnish the room with personal items for added comfort. Admission documents will need to be completed, either by you or a designated individual handling your affairs.
